Terms of Service
Terms of Service Agreement
This Agreement is entered into by and between Rejuvenating Health LLC (“Rejuvenating Health,” “Company”) and the undersigned client (“Client”). Rejuvenating Health and Client may be referred to individually as a “Party” and collectively as the “Parties.”
RECITALS
A. Ownership & Expertise
Lindsey VanSchoyck and Matt VanSchoyck are the owners of Rejuvenating Health and possess extensive education, clinical training, and professional experience in health optimization, functional medicine principles, lifestyle medicine, nutrition, and health coaching.
B. Proprietary Systems
Rejuvenating Health has developed and owns proprietary systems, methods, educational materials, protocols, and processes related to health assessment, lifestyle optimization, nutrition education, coaching support, and implementation strategies.
C. Services Offered
Rejuvenating Health provides health-focused education, coaching, and support services designed to help clients identify root causes of health concerns, improve lifestyle behaviors, and support long-term health outcomes. Services may include clinical-style lab review education, lifestyle planning, nutrition guidance, health coaching, and ongoing support.
D. Client Engagement
Client voluntarily seeks services from Rejuvenating Health and acknowledges the Company’s reputation, experience, and approach to health optimization.
E. Mutual Agreement
Client desires to engage Rejuvenating Health for health coaching and educational services, and Rejuvenating Health agrees to provide such services in accordance with the terms of this Agreement.
ARTICLE I – SCOPE OF SERVICES
1.1 Commencement of Services
Services shall begin following receipt of the Client’s initial payment and, when applicable, the ordering of laboratory testing.
1.2 Nature of Services
Rejuvenating Health provides non-emergency, non-primary care, educational and coaching-based services, which may include but are not limited to:

Functional and lifestyle-oriented review of health history and symptoms

Education surrounding laboratory data and health markers
-
Personalized lifestyle medicine recommendations (nutrition, movement, sleep, stress management, and behavior change)

Health coaching support for implementation and accountability
-
Access to educational resources, proprietary portals, and tools
-
Group and/or individual coaching calls
-
Messaging or platform-based communication as outlined in the purchased program
Rejuvenating Health services do not replace primary care, specialty medical care, or emergency services.
1.3 Duration of Services
The duration and structure of services are defined by the specific program or package purchased by the Client. Details regarding frequency, access, and support are outlined in the applicable product description or invoice.
1.4 Client Participation & Progress
Client acknowledges that meaningful health improvements require active participation, including but not limited to:

Completing labs and assessments when ordered

Attending scheduled coaching sessions or reviews

Implementing recommended lifestyle and nutrition changes

Communicating challenges and progress honestly
Rejuvenating Health commits to continuing to work with the Client through the agreed program when the Client is actively participating in all aspects of care, recognizing that timelines for improvement vary by individual.
1.5 No Guaranteed Outcomes
Rejuvenating Health does not guarantee specific results, including but not limited to weight loss, symptom resolution, or disease reversal. Outcomes depend on multiple factors, including Client participation, consistency, medical history, and individual response.
1.6 Independent Contractor Relationship
This Agreement does not create an employment relationship, partnership, joint venture, or ownership interest between Client and Rejuvenating Health or its owners.
ARTICLE II – FEES & PAYMENT
2.1 Fees
Client agrees to pay the total program fee as outlined in the invoice or payment agreement selected at enrollment. Payment may be made in full or via an approved payment plan.
2.2 Payment Method
The initial payment is due at the time of execution of this Agreement. Subsequent payments will be processed using the payment method on file and must be updated if no longer valid.
2.3 No Refund Policy
Due to the nature of services, including immediate access to proprietary materials, clinical preparation, and laboratory ordering, all payments are final and non-refundable.
2.4 Chargebacks
Client agrees not to initiate chargebacks or payment disputes. Any chargeback constitutes a material breach of this Agreement and may result in immediate termination of services, acceleration of outstanding balances, and recovery of associated fees, including reasonable collection and legal costs.
ARTICLE III – DISCLAIMERS & GENERAL PROVISIONS
3.1 Disclaimer of Guarantees
Rejuvenating Health makes no warranties or guarantees regarding outcomes. Client acknowledges that health results vary and that education, coaching, and support do not constitute a promise of specific medical or physical results.
3.2 No Medical Emergency Care
Services provided are not intended for diagnosis, treatment of emergencies, or urgent medical conditions. Client agrees to seek appropriate licensed medical care when necessary.
3.3 Assignment
This Agreement may not be assigned without prior written consent from Rejuvenating Health.
ARTICLE IV – CLIENT RESPONSIBILITY & SHARED SUCCESS
4.1 Educational & Coaching-Based Services
Client acknowledges that Rejuvenating Health provides education, coaching, guidance, and lifestyle-based recommendations rooted in functional and lifestyle medicine principles. Services are designed to support health optimization, root-cause awareness, and sustainable behavior change and do not constitute medical treatment, disease diagnosis, or guaranteed outcomes.
4.2 Client Responsibility for Implementation
Client understands and agrees that progress and outcomes are directly dependent on the Client’s active participation, including but not limited to:

Completing ordered laboratory testing and assessments in a timely manner

Attending scheduled coaching sessions, reviews, or check-ins

Implementing nutrition, lifestyle, and behavioral recommendations as discussed

Communicating challenges, barriers, and changes honestly

Engaging consistently with educational materials and support resources
Rejuvenating Health provides the roadmap, education, and professional support; however, implementation remains the sole responsibility of the Client.
4.3 No Guaranteed Results
Client acknowledges that individual responses to lifestyle, nutrition, and health coaching interventions vary significantly. Rejuvenating Health makes no representations or guarantees regarding symptom improvement, weight change, lab values, disease outcomes, or any specific health result.
4.4 Ongoing Support with Active Engagement
When the Client is actively participating in all agreed components of the program, Rejuvenating Health is committed to continuing support, education, and refinement of recommendations within the scope and duration of the purchased program. Progress timelines may vary based on individual physiology, consistency, and external factors.
4.5 Not a Substitute for Medical Care
Client understands that Rejuvenating Health services are not a substitute for primary care, specialty care, mental health services, or emergency medical treatment. Client agrees to consult appropriate licensed healthcare providers for concerns beyond the scope of coaching and education.
4.6 Assumption of Responsibility
Client voluntarily assumes full responsibility for health decisions, lifestyle choices, and actions taken based on information provided by Rejuvenating Health. Client agrees that Rejuvenating Health, its owners, employees, and contractors are not liable for outcomes related to the Client’s choices or level of participation.
